How to apply for an electronic health card for the first visitors?

Please register and apply for an electronic health card on our official WeChat account, and the registration number is the medical certificate.

Patients who are not registered on WeChat official account can take their ID cards or medical insurance cards to the registration window to create a card, or create a card on the self-service machine in our hospital. Infants without ID number can use their birth certificates to create a card at the registration window.

Registration

1. Registration Time

Both online and offline registration are available one week within the visiting time, with an appointment period of 7 days. Times for different ways of registration:

(1) Hospital Official WeChat Account:

Daily: 6:30-23:00

(2) Self service machine:

Daily: 6:30-17:00

(3) Telecom 114 appointment: the appointment can be made every morning from 5:30 to 6:30 (decided by Telecom 114 appointment platform)

(4) Hospital Registration Window, Daily: 7:30-21:00

2. Precautions

(1) Apply for an electronic health card with passport .

(2) Before registration, please carefully select the department, and doctor. You can ask for advice at the information desk. When making an appointment online, please carefully read the precautions and related prompts. If the appointment is abandoned due to the individual's own reasons, the fee will not be returned.

(3) After registration, please pay attention to the visiting time, location, department, diagnosis room, and doctor's name, and visit in a timely manner.

(4) When your telephone number is changed, please go to the registration window and tell the working staff in a timely manner to ensure normal use.

(5) The outpatient examination fee (including registration fee) is as following standards:

VIP Consultation: 300 CYN/person, 200 CYN/person, 100 CYN/person

Famous Doctor’s Consultation: national TCM masters, experts enjoying special allowance granted by State Council: 90CYN/person;

Sichuan Top Ten TCM Doctors : 80 CYN/person

Sichuan Famous TCM Doctors: 70 CYN/person, 50 CYN/person, 30 CYN/person;

Chief physician: 21 CYN/person;

Associate chief physician: 17 CYN/person

Attending physician: 13 CYN/person

Emergency: 18 CYN/person

According to document No. 81 of 2005, the cost of gynecological examination is 5 CYN/person, otolaryngology is 2 CYN/person, obstetrics is 10 CYN/person, and registration is required

Canceling registration: If you need to cancel the appointment , please cancel it one day before the visit. If your cancellation reaches 3 times within one month, the electronic card cannot make an appointment for 3 months.
